We retain sources and checked dates, and record verified changes in a change log.

We review errors or updates concerning identity, clinicians, procedure availability, price or review observations, and source links.

How changes are handled

We compare the original source and checked date again before updating. Unverified requests do not overwrite the evidence state.

Record keeping

When a published result changes, its reason, effective date, and affected snapshot are recorded together.
